Choose To Change (C2C)
In PartnerShip with Youth Advocate Program (YAP) and BrightPoint
At the core of the C2C model is the combination of intensive youth wraparound services and advocacy along with a trauma-informed group therapy called SPARCS (Structured Psychotherapy for Adolescents Responding to Chronic Stress).
Lifeline to Hope serves 200 youth per year under this program.

Scaling Community Violence Intervention for a Safer Chicago, or “SC2
Lifeline To Hope (L2H) works in the West Garfield Community. L2H provides street outreach services, life coaching, trauma treatment, education, and job training. We share data, as well as make and receive referrals to our partner organizations: Breakthrough, INVC, Black Men United, Together Chicago, and other community stakeholders. In addition, we collaborate to identify potential conflicts, mediate disputes, and negotiate peace treaties.
Chicago Housing Authority - Summer Youth Employment Program
(CHA - SYEP)
SYEP is a paid work and enrichment experience for youth, including financial literacy, job development workshops and young adult mentors. L2H has a summer youth program focused on youth development and equitable opportunity. We were selected by CHA to become one of their summer employment partners, focusing on the CHA residents on the Northside of Chicago; working to connect them to meaningful work experiences.
